Criminal Background

The Pennsylvania Department of Education requires that all Health Care Employees in the state of Pennsylvania have clear criminal records. As a result, in order to save potential students time and expense in getting a Certification that they won’t be able to use because of something in their background, we require all applicants to  the CNA program to have  a background check that shows they are free of felonies or prohibited offences. You can submit the application yourself directly to the state police for a fee of $22.00 or we can submit it for you for $30.00. If you chose to do it yourself, we need to receive the original State Police report from you before you can start classes. Please note  a

If you would like us to do it for you, please indicate that on your application.  A fee of $30 will be automatically added to your invoice. Please note, once we run the check, this fee is non-refundable regardless of the outcome. If you chose to submit the Background check request directly to the state police, make sure that you submit the required SP 4-164 which can be found on the police website. No other form is acceptable.

To get a background check by the Pennsylvania State Police, you must also complete a “Verification of Pennsylvania Residency” form. If you are getting your check done by yourself, you will find the form on the Police site.

NOTE: If you have lived in the State of Pennsylvania for less than two years, you will have to submit to an FBI background check.

You can register with the state by logging on to their website and receiving a list of recognized places to do the process with you. You will have to use our school code which is 1KG6XN INDIGO.

We believe a simpler way is to contact Rosa Fingerprint  and let them do all the work. PHILADELPHIA ACADENY for NURSE AIDE TRAINING is NOT affiliated with this organization and receives NO benefit from them.
